This is the authorization between you and Peace of Mind to allow us to come into your home and service your animals.

In the event of any health issue, including an emergency, we will always try to contact you first. This document is to help guide us towards making appropriate medical decisions you are comfortable with in the event we cannot reach you for some reason. It’s never a pleasant thing to think about, however a veterinary release definitely falls into the ‘better to have it and never need it than need it and not have it’ category.
